    2. Both players will be paid for when signing up on line. A $2.00 service charge has been added for a total of $72.00. 
  2. When the transaction is completed you will be sent back to this page.
  3. Please fill out the information form.  Ignore "Email Address" if we already have it.
  4. On the bottom of the page click  "Submit Form".  When you do that, the information you provided will be saved.  You will be sent a confirmation email within 24 hours.
